Nothing sits in a case; your chain is cut and your clasp is attached only once your order comes in.
Before you order.
Wrap a soft tape (or a string measured against a ruler) around your neck at the height you want the chain to sit, then add a little slack for comfort. The number you land on is the total finished length, clasp included.
Because nothing is pre-cut, expect more lead time than an off-the-shelf piece — the chain is cut to your exact inch and the clasp is fitted before it ships.
Yes, up until your chain is cut and finished. Once it's in production the clasp is soldered or fitted to the chain end, so changes after that point mean starting a new piece.
